Hartog House provides onsite case managed support services. As part of the transitional program, individuals identify short and long-term goals. These goals are aimed at helping individuals to transition to long-term and permanent housing, self-sufficiency and independence. Hartog House uses the wrap around model of support and relies heavily on other community agencies for additional support services.
